Overview
Enterococci are used as alternative faecal indicator organisms alongside E. coli, particularly in bottled water and water intended for infant formula preparation. They are more resistant to environmental stress than E. coli.
Health Relevance
Most enterococcal species are harmless to healthy adults. Enterococcus faecalis and E. faecium are important opportunistic nosocomial pathogens — particularly Vancomycin-Resistant Enterococci (VRE), a critical AMR concern in hospitals.

Controversy & Contested Science
VRE in environmental water bodies creates a reservoir for horizontal resistance gene transfer to other bacterial species. Hospital wastewater — rich in VRE — is typically discharged into municipal sewer systems without additional treatment, potentially seeding resistance genes into the urban water cycle. A 2020 EU AMR Action Plan update identified environmental water pathways as underaddressed transmission routes for resistance genes — a gap with direct implications for clinical antibiotic efficacy.
