Overview

Heroin (diacetylmorphine) rapidly metabolises to morphine in the body. Morphine and conjugates are the primary excretion products used in WBE to indicate heroin use. Stockholm and Sweden broadly have lower heroin WBE signals than Western European counterparts.

Health Relevance

At environmental trace concentrations, heroin metabolites have no pharmacological effect. Their presence is a public health surveillance indicator, not a direct risk.

Regulatory Limits

Drinking Water Directive 2020/2184

No regulatory limit.

Controversy & Contested Science

Sweden operates one of Europe's most restrictive drug policies — 'zero tolerance', no drug consumption rooms, punitive sentencing. EMCDDA WBE data show Sweden with lower heroin but high amphetamine concentrations relative to Western Europe. Wastewater epidemiology provides an objective metric for evaluating policy effectiveness, bypassing self-report biases — and has been cited in academic evaluations of Scandinavian drug policy. Critics of the Swedish model point to high overdose mortality rates and HIV transmission costs despite apparently lower prevalence.
